Billing & Insurance

I may be in network with your insurance provider(s), and I also accept direct or out-of-pocket payment. I have a limited number of sliding scale appointments available; please ask about my sliding scale availability.

Insurance I am currently in network with include:

  • Blue Cross/Blue Shield PPO

  • Optum/UnitedHealthcare PPO

  • Evernorth (Cigna)

  • Aetna

For insurance providers with whom I am out of network, I do not submit billing on your behalf. Instead, I will provide a detailed invoice at the end of each month for you to submit to your insurance provider for them to reimburse you directly. If you have insurance with which I am not in-network, I encourage you to contact your insurance provider to ask about their policy for covering out-of-network services.

If you are paying out of pocket (self-pay, sliding scale, and/or out-of-network insurance coverage), I will provide you an estimate (Good Faith Estimate) of the cost of services prior to beginning work together. I am happy to discuss financial matters whenever questions or concerns arise.

Regardless of whether you choose to use your in- or out-of-network insurance benefits, or pay out of pocket, I will bill for our appointments at the end of each month.
